This is a full listing of all the Patient file fields. There are over 700 and the vast majority are
specific to VA functionality that is likely not found outside the Veteran Administration.
Fields shown in
italics are fields designated as "VA SPECIFIC" and may
function without complications, but very well might have triggering or control
issues that could complicate the use of your template that you are building.
In terms of demographics for the patient file and the database, there are a
a number of fields that are mandatory to load a patient:
- Name (Client's Lastname,Firstname MI - note that there is no space after
the comma)
- Sex (M for male, F for female)
- DOB-Date of birth, which can take the following forms and can only
be in the past tense:
- A year of birth (1947, always notate the full year)
- MM/YYYY (03/1947)
- DD/MM/YYYY (03/29/1947)
- DD/MM/YYYY@HH:MM (03/29/1947@14:23)
- T-10 (Translates to "TODAY minus 10 days")
- T-3W (Translates to "TODAY minus 3 weeks")
- T-2M (Translates to "TODAY minus 2 months")
- T@00:33 (Translates to "33 minutes post mid-night")
- ID (Primary identification for the facility/region/country)
- ID : This can be up to 50 alpha-numeric characters
- SSN: A U.S. national ID. Either in the form of NNNNNNNNN or
NNN-NN-NNN
- HRN: This is Health Record Number, typically used by provider systems
for their own internal unique identification
- There are several other mandatory fields required by the VA, but these are
automatically filed by the application and will have no impact on your local work.
Using the Find Field: For best results, I think, a single word will get you
the most concise list. However, you can enter multiple words separated by a comma. Due to the
similiarity in syntax in the field labels, you will likely get a large list. The fields will
be listed in the memo pad, which is an open ascii type text area where you can cut/copy/paste,
etc.
